    Elder and Disabled Adults Programming
    The Need

    Increasing the availability of services to elderly and disabled adults is a priority of the CCNTR Board of Directors. Lack of resources such as transportation, and mental health programs have had a negative impact on this segment of our population, as has isolation from the larger community.

    Actions
    An outreach program staffed by a social worker, helps elders and disabled adults, living in isolation, access services they need. This program was developed by a cross-agency care coordination group. The ServiceLink program, a 24 hour telephone information line, also specializes in helping elders and their families connect with services in the region.

    Involvement
    CCNTR has been able to fund staff positions at two community-based agencies - Newfound Area Nursing Association and the Visiting Nurse Association of Franklin. This inter-agency access manager has been instrumental in helping community members receive needed services. This work is funded through Healthy Communities Access Grant, Rural Health Network Development grants and Medicaid fees.
